Make the chart fit your family
Write a name, week beginning date and one goal at the top of the printed chart. The five rows are available spaces, not instructions to schedule five daily attempts. Label only the rows you want to use and leave the rest blank. Agree on what a mark means so two caregivers can record the same kind of event consistently.
Keep notes without making predictions
Use the second PDF page or the Excel record for dates, times and brief observations in your own words. The files do not calculate readiness, success rates or a deadline for learning. They are blank records rather than medical guidance. Keep personal notes private, print a fresh chart when needed, and choose the paper or typed version that is easier for your household to maintain.
